A Family Guide to the Transition-Focused IEP

The Tennessee Department of Education has released a new resource called A Family Guide to the Transition-Focused IEP. This guide was created in partnership with Transition Tennessee, Family Engagement in Special Education, and the TN-TAN network to help families of children with disabilities understand how to plan for their child’s life after high school.

The guide explains what a transition plan is and how it becomes part of a student’s Individualized Education Program (IEP) starting at age 14. Families can learn about measurable postsecondary goals, transition assessments, courses of study, annual goals, and outside agencies that can help after graduation. It also includes examples, checklists, and answers to common questions to make the process easier to follow.

Transition planning helps students think about their future education, career, and independent living goals while still in high school. The guide is a great place for Tennessee families to start learning about these steps.

Every student’s journey is different, and some families may still need help understanding how this process applies to their child. Families can reach out to the Family Engagement in Special Education team for personalized transition support. Specialists are available to answer questions, explain parts of the IEP, and connect families to programs and services across the state.
